- Change Enforceable Payment Due Date – Used by the creditor to ask the Maintenance Enforcement Program (MEP) to accept the debtor's payments on a date later than the due date set out in the court order.
- Child Status Report – This form provides information to MEP about a child’s status to confirm their eligibility for maintenance.

- Client Authorization of Third Party – This form is used by a client to authorize another person to receive information about their MEP file.
- Creditor Direct Deposit – This form gives MEP permission to deposit funds directly into a creditor’s bank account (click link for details). Attach with this form a copy of a void cheque OR bank account information from the bank or financial institution with a bank stamp. Email the completed form and attachments to albertamep@gov.ab.ca.
- Registration Request – Maintenance Enforcement Program – Use this form to request registration with MEP for collection of court-ordered child, spousal and partner support payments.
- Affidavit of Arrears – This form is used to claim arrears. This is required if you wish to register with MEP and the debtor has missed payments in the past.
- Section 7 Expenses Agreement – This form is used to make an agreement to clarify the types of section 7 expenses to be shared. This form is needed for MEP to enforce expenses if the court order requires the parties to pay a percentage or proportionate share of expenses for the children, but does not specifically detail which expenses are to be shared.

- Section 7 Expenses Information and Report – This package provides information for clients about how MEP administers Section 7 expenses. The package also includes the form that clients need to use, to submit Section 7 expenses to MEP for reimbursement by the other party. This package was updated according to changes made to MEP's procedures regarding Section 7 Expenses.
- Substitutional Service Request Form – This form is used to request that MEP serve court documents relating to any legal matter if both things are true:
- The respondent is registered with the MEP.
- The MEP has an address for service for the respondent.
- Lost Cheque Statutory Declaration – This form is used by clients to request a replacement cheque from MEP because they have either lost or not received the original cheque.
- Maintenance Enforcement Support Agreement – This form is used to make an agreement for child support, spousal support (for married couples), adult interdependent partner support, or to change the terms of an existing Maintenance Enforcement Support Agreement. MEP can enforce certain maintenance agreements made between parties.
- Maintenance Enforcement Support Agreement – Instructions – These instructions will help you complete the Maintenance Enforcement Support Agreement form.
- Renewal of Interest – This form is used by creditors who previously did not wish to have interest charged and collected on their file, but who now wish for MEP to collect interest on their behalf.
- Request to Lower the Amount of Maintenance or Arrears Collected – This form is used by creditors to request that MEP collect a lower amount of maintenance or arrears than that in their maintenance order or agreement.
- Withdrawal request – Only the party who asked to be registered with the Maintenance Enforcement Program can request to withdraw from the program.
