Overview
Community access for people in continuing care (CAPCC) helps adults with disabilities in facility-based continuing care engage with their communities to reduce loneliness and social isolation.
You are eligible to receive supports from this program if you are:
- an adult Albertan under the age of 65
- living in type A or type B continuing care home
- able to actively participate in establishing and following through with your goals and able to communicate your preferences
Community access coordinators can help you reduce loneliness and social isolation by:
- becoming more active in your community
- socializing with your family and friends
- developing an individual service plan, based on your interests and available community activities, such as:
- outings with family and friends
- volunteer activities
- church or spiritual activities
- movies
- continuing education
