Notification letters
Levels of consultation require their own degree of notification. The templates available below are intended to help proponents connect and specify to First Nations or Metis settlements, or both, which of the 3 levels of consultation is being initiated.
First Nation notice letters

PDF form issues
Fillable PDF forms do not open properly on some mobile devices and web browsers. To fill in and save the form:
- Save the PDF form to your computer – click or right-click the link and download the form.
- Open the PDF form with Adobe Reader. Fill it in and save it.

Record of consultation log
Proponents will maintain a complete record of consultation efforts using the provided Record of Consultation (ROC) Log template:
- Record of Consultation Log (8.5 x 11) (PDF, 3.3 MB) effective as of December 2020
- Record of Consultation Log (11 x 17) (PDF, 2.4 MB) effective as of December 2020
Consultation log review
Once consultation is concluded, proponents are required to complete the ROC log and, when required, send it to the First Nations or Metis Settlements, or both, for review of errors or omissions. Refer to the December 2019 Proponent Guide (PDF, 2.0 MB) and the 2019 Proponent Guide: Overview of Major Changes (PDF, 311 KB) for more information.

Concern and response table
The Consultation Concern and Response Table (PDF, 2.0 MB) is used at the direction of a government Consultation Advisor to document specific concerns raised regarding project consultation.
Supplemental information
Pre-Consultation Assessment Requests that contain activities regulated under the Water Act or the Environmental Protection and Enhancement Act (EPEA), or both, whether or not they are in combination with activities regulated under the Public Lands Act, must include a Water Act/EPEA supplemental form (PDF, 2.9 MB) providing additional details in the fillable document.
Pre-Consultation Assessment Requests that contain activities regulated under the Forests Act must include a Forestry Activities Supplement Form (PDF, 3.1 MB) providing additional details in the fillable document.
