Overview
Children’s Services internally reviews every death or serious injury of a child receiving intervention services. As part of this work, the department will conduct designated reviews when needed to provide new findings and recommendations that can further improve the system.
Designated reviews operate in consideration of those reviews done by the Office of the Child and Youth Advocate and others, avoiding unnecessary overlap and focusing on understanding what happened and where programs, policy and practice need to be strengthened.
The goal is to prevent or reduce the risk of future deaths and lead to better outcomes for children, youth and families. Read more about deaths, serious injuries and incidents of children receiving services.
Designated reviews
Findings and recommendations are publicly released for each review:
- Deaths of two young children
- Criminal charges of caregivers
- Two young children harmed in kinship care
- A six-month-old-who died in parental care
- Injuries causing death following return to familial care
- Safe sleep-related deaths in parental care
- A 16-year-old who died by suicide
- Physical abuse and neglect of two young girls
