- Client Consent form – As of July 1, 2022, the Client Consent form has replaced the Client Declaration form. The Client Consent form will be accepted in any situation that calls for the Client Declaration form. Authorizers, specialty assessors, specialty suppliers are expected to submit the Client Consent form with every authorization and save a copy on the client’s file moving forward just as they did with the Client Declaration form.
