Find a participating registry agent where you can apply for or update your AHCIP health care card information.
You can bring your Alberta Health Care Insurance Plan (AHCIP) forms to a participating Alberta registry agent office. When you go to the registry agent office, bring your original supporting documents with you.
Pick up AHCIP forms at the registry agent office, or download the AHCIP forms and complete them before you bring them to a registry agent office.
Not all registry agent offices in Alberta provide AHCIP services, only those listed below.
Registry agents cannot access your personal or medical information. Your personal information is safe.
Alberta Motor Association (AMA) only provides services to their members.
