Alberta’s government launched a convenient card-sized proof of vaccination that is available easily and quickly through alberta.ca/CovidRecords. Starting Sept. 21, and over the following days, Albertans will be able to visit a local registry agent to request a printed version of their vaccination card at no cost. They will need to provide the month and year of any dose of vaccination, their Alberta Personal Health Number and their date of birth.

A printed version of the card will make it easier for Albertans to show their vaccination status to businesses checking patrons for government-issued proof of vaccination under the Restrictions Exemption Program. Albertans seeking a printed version from a registry agent should contact their registry agent first to confirm the service is available.

Temporary health measures are now in effect for restaurants, weddings and funerals, retail, entertainment venues and adult sport and fitness to help slow the spread of COVID-19.

Starting Sept. 20, eligible businesses and events must either implement the Restrictions Exemption Program requiring proof of vaccination, a privately paid negative COVID-19 test result or a valid medical exemption letter from a physician or nurse practitioner to continue operating as usual, or adhere to all public health measures now in effect.

Albertans are able to use their existing immunization records, including those received at their vaccination appointment or saved from MyHealth Records, after the proof of vaccination requirements are implemented on Sept. 20. 

Work is also underway to make proof of vaccination available through a QR code. A QR code will be an easier, faster and more secure way to share the immunization record when needed. The QR code is expected to be available in the coming weeks.

Eligible Albertans who have not yet received their vaccinations are reminded to book their appointments as soon as possible.

  • Albertans have several options to access and present their COVID vaccination records:
    • Visiting Alberta.ca/CovidRecords and entering the month and year of their vaccinations, their Alberta Personal Health Number and their date of birth.
    • Presenting their vaccination records previously received through their MyHealth Records account.
    • Presenting the paper immunization records received in person following their COVID-19 vaccination appointment.
    • Contacting the pharmacy or physician's office where they received the vaccine or calling 811 to request a copy to be mailed.
  • COVID-19 vaccination records may take up to two weeks to show on both alberta.ca/CovidRecords and MyHealthRecords. It depends on when and how the health practitioner inputs the information after administering the vaccine.
