Overview

You must get a background check done for the OPGT if you’re applying to become a:

  • guardian of an adult
  • trustee
  • co-decision maker

This background check is made up of the following:

  • criminal record check
  • credit check (only for trustee applications)
  • reference check
  • identity verification

Get a background check

Step 1. Fill out the forms

If you want to verify your identity with the OPGT or a community group

Fill out the forms in the OPGT Background check package (PDF, 142 KB):

  • Privacy Notice and Consent
  • Data Collection
  • Supplementary Data Collection and Consent

For the Supplementary Data Collection and Consent form, 2 of your references must match the ones you provided in your application package on Form 30: Personal References (PDF, 30 KB).

You should inform your references they will be contacted by phone.

Step 2. Gather accepted ID

Two pieces of ID are required to complete a Canadian criminal record check.

One piece must be a current government-issued photo ID which includes your:

  • name
  • date of birth
  • signature

The second piece of ID must include your full name.

Accepted ID

  • driver’s licence
  • provincial ID card
  • passport
  • Canadian citizenship card
  • permanent resident card
  • Indian status card
  • government ID card
  • municipal employee ID card

Not accepted ID

  • SIN cards
  • credit card
  • debit card

Step 3. Have your identity verified

To verify your identify, you’ll need your:

  • 2 pieces of accepted ID
  • completed forms from step 1
