Where to apply for AHCIP

Participating registries

You can take your Alberta Health Care Insurance Plan (AHCIP) forms to a participating registry. Not all registries in Alberta provide AHCIP services, only those listed below.

When you go to the participating registry, bring your supporting documents with you.

• Participating registries process AHCIP and non-group applications, update AHCIP information (for example, name, address, marital status) and order replacement cards.
• Processing time can take up to 5 days.
• Health care cards are mailed to your home from the AHCIP office.
• This is a free service.

You can pick up AHCIP forms at the registry, or download the AHCIP forms and complete them before taking them to a registry.

Registries cannot access your personal or medical information. Your personal information is safe.

• Only specially trained health agents can help you.
  • Contact the registry in advance, to ensure they have health agents available to assist you.
• Registries are not authorized to give you your personal health care card, it will be mailed to you by the AHCIP office.
  • Request the registry confirm your mailing address is correct in the AHCIP system.
